Admission Procedure Guidelines

Fill-up Triage Check List
Present Triage checklist at Information desk inside the Clinic
(if HMO member, present HMO ID and One Valid ID)
List of Acceptable IDs (any of the following):
- Accredited HMO-issued member’s ID
- Senior Citizen’s ID
- PhilHealth ID
- Persons with Disability ID
- Healthcare insurance card or document
- Digitized SSS ID
- GSIS ID
- PRC ID
- Unified Multi-Purpose ID
- Driver’s License
- Passport
Admitting Section for settlement of requirements
The patient will be asked to sign consent forms for hospital care:
- Authorization for test, procedure & treatment, Authorization for emergency treatment, Undertakings, Authorization for release of professionals and medical record, Authorization to administer medicine from Clinic pharmacy/ Release from liability if sourced elsewhere, Release from responsibility for valuables, Warranty of Payment.
- Signed Letter of Authority (LOA) / Referral from your company (if you are an eligible employee for company healthcare hospitalization benefits)
- Any prior medical records, X-Ray films or test results (especially if you had a recent hospital confinement at another medical facility or hospital)
Patients are referred to the Emergency Room (ER) if they do not have an admission order sheet from their attending physician. In such cases, the ER consultant will ask the patient for the name of his/her physician. The ER consultant may only assign an attending physician to the patient’s case if he or she does not have a personal physician of choice.
HMO Holder Guidelines

Check whether your Health Maintenance Organization (HMO) is accredited by Protacio Health Before admission present your HMO Identification Card (ID), coordinate with your HMO so that all documents can be prepared in advance to prevent unnecessary delays and inconvenience at the hospital’s admission office.

PhilHealth Guidelines

If you are a PhilHealth member, you will be provided a list of requirements and forms required to avail yourself of PhilHealth deductions from your final bill. You or a family member can work on these immediately upon admission to avoid delays during discharge.
Steps in Accomplishing PhilHealth Requirements
Kindly accomplish the following:
- Claim Signature Form (CSF)
Generated PhilHealth Benefit Eligibility Form (PBEF)
- If the PBEF indicated YES – no need for CSF
- If the PBEF indicated NO – accomplished CSF by employer and other required document indicated at PBEF
Certificate of contribution or Receipt of Payment
- Three (3) months before admission
Properly accomplished PhilHealth Member Registration Form (PMRF)
